Tell us which tests you plan to order, who may authorize testing, where results should be sent, and how your company should be billed. We will send you our Agreement for Services to complete.

Employer account inquiry
Tell us what your company needs.
Include your company name, contact phone number, testing needs, and approximate monthly testing volume in the message. We will follow up with the Agreement for Services and account setup details.
Please do not submit employee identifiers, test results, or medical records. This form requests account setup; it does not activate an account.
After your account is active
A consistent process for every employee.
Your authorized contact sends the testing order, the employee completes the collection, and we report the result according to your account instructions.
Collection
All drug testing is available on a walk-in basis during regular office hours.
Results
Results are emailed or routed only to the authorized recipient established for your account.

Can applicants walk in without an appointment?
Yes. All drug testing is available on a walk-in basis Monday through Friday from 7:30 AM to 3:30 PM.
Can we be invoiced?
Qualified employer accounts may arrange billing under the terms of the completed Agreement for Services.
How are results delivered?
Results are delivered to the authorized company contact listed in your account instructions.
Can you manage a DOT random pool?
Yes. We provide C/TPA services for qualifying regulated employers and owner-operators.
